Thank you for your interest in the Penn Psychiatry TEAM Clinic. We provide outpatient mental health services for individuals experiencing mild to moderate mental health symptoms, including depression and anxiety-related concerns. Our team includes therapists and medication providers who work collaboratively to offer evidence-based treatment, including 4 months of goal-oriented therapy, medication management, or a combination of both. Care is delivered by highly trained licensed psychotherapists, psychiatric nurse practitioners, and psychiatrists as part of a multidisciplinary team.

This service is not appropriate for:

- Worker's compensation

- FMLA (Family and Medical Leave Act) requests

- Documentation for disability requests, emotional support animals, legal proceedings or school/personal accommodation requests

- Motor Vehicle Accident Claim

- Forensic evaluation

- Disability paperwork

- Neuropsych testing (e.g. ADHD)

This brief survey helps us learn more about your current needs. After reviewing your responses, one of our patient service associates will reach out to discuss next steps. If appropriate, we'll offer an initial in-person evaluation with both a therapy and medication provider. This appointment is a chance to explore your concerns further and identify a treatment plan that fits you. Telehealth options for ongoing care can be discussed with your providers at that time.
